1. Complete the account form on our website. (All forms can be found on the FORMS PAGE.) 

 

2. Complete the case preferences form. EMAIL both forms to jennifer@hermitagelab.com or FAX to 615-889-4953.

 

3. If a digital case, submit digital cases through the designated portal.

 

4. Fill out online RX in the Forms tab for non-digital cases.

 

5. For traditional impressions in the Nashville area, schedule a pickup with our local driver by calling 615-889-4949. Calls placed after 8am may be scheduled for the next day depending on location.

• For locations that we pick-up through UPS you can print a label on our website by clicking “send a case” tab. Choose New HDL Client and fill out shipping information, print label and attach to case box.
